Request Images/Reports

For your convenience you may request images or reports by completing and submitting our online Medical Records Request/Authorization form.

• Complete the Medical Records Request/Authorization form
• Fax the completed form to 702-598-1094
• Pick up requested materials at selected Desert Radiologists facility

Please allow 48 hours for processing of all image/report requests. Proper proof of identification must be provided in order to pick-up medical records.  Please be advised that you may encounter a minor wait when you arrive at our faciity to complete your medical records request.
